Responsibilities

Position Summary:

As part of the SUD-PC team, facilitating access to treatment for persons with substance use disorders (SUD) and co-occurring mental health disorders (COD). This position provides services throughout the URMC network, rotating inpatient and outpatient. This position conducts screening and assessment for individuals seeking treatment and making appropriate referrals for all levels of care. This position provides direct clinical services for adults, young adults and adolescents.

Responsibilities:

• Facilitating access to treatment for persons with substance use disorders (SUD) and co-occurring mental health disorders (COD).

• This position conducts screening and assessment for individuals seeking treatment and making appropriate referrals for all levels of care. Enhanced skills in interviewing and engaging patients and diagnosing substance use disorders using the DSM-V

• Provides direct clinical services for adults, young adults and adolescents.

• Assists patients through the admission process to treatment. Provides assessment and individual therapy treatment for patients. Participates in safety planning and treatment plan development. Performs BAC tests and Narcan training as needed.

• Manage incoming referrals from the initial point of contact. Tasks include chart review, gathering and reviewing information from referring providers and patients, accessing work queues, conducting phone screens, scheduling patients, relaying pertinent information to providers involved in the care, searching databases such as ISTOP, conducting reports and attending team meetings. Works closely with Suboxone providers in the stabilization program. Requires strong skills in record keeping and communication.

• Provider consultation, discussing best level of care for patients and treatment plan, reviewing controlled substances tapers, coordinating Medicated assisted treatment plans. Collaborating throughout the patient's care with providers on patients progress.

• Ensures compliance with all applicable regulatory and institutional requirements and standards. Ensures the integrity and security of patients at all times. Initiates, monitors and ensures that all documents are standardized and within OASAS and Joint Commissions guidelines. Implements immediate and appropriate corrective action when inconsistency of activities is identified.

Other duties as assigned

Qualifications:

• Master's Degree in a discipline related to human services required.

• 1+ year of related experience in substance use disorder treatment setting required;

• or an equivalent combination of education and experience.

• Possesses skills in interviewing and engaging patients and experience in diagnosing substance use disorders using the DSM-V. Demonstrates a working knowledge of various levels of care, referral processes and medication-assisted treatment preferred.

• Certified as a NYS Credentialed Alcoholism and Substance Abuse Counselor (CASAC) or (CASAC-T). If CASAC-T upon hire successful completion of examination to remove the (T (training)) designation must be completed within 12 months of hire required.

Pay Range

Pay Range: $ 57,595 - $ 80,600 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
