    The UW MEDICAL CENTER - MONTLAKE - SOCIAL WORK DEPARTMENT has an outstanding opportunity for a PART-TIME SOCIAL WORKER - RADIATION ONCOLOGY (80% FTE, Day Shifts) . The Radiation Oncology Clinic Social Worker functions as a core member of the Radiation Oncology interdisciplinary care team. This position will provide both practical and psychosocial services to oncology patients and their support systems during their radiation treatment course, to include: conducting initial needs assessments of psychosocial functioning, resource identification, access issues and emotional support of patients and their caregivers while preparing for and receiving radiation treatment. The Radiation Oncology Social Worker will provide real time crisis intervention for patients, and consultation with team members and associated medical specialties throughout treatment course on scheduled work days.

    PRIMARY RESPONSIBILITIES: * Assist in connecting patients with available resources for transportation, lodging and financial support to decrease barriers to accessing radiation treatment. * Collaborate with inpatient and oncology social workers to ensure smooth transitions between care modalities. * Utilize medical record and consultation with interdisciplinary staff to screen patients for service needs, provide information regarding Social Work activity and to coordinate care as part of an interdisciplinary team * Assess for the psychosocial and logistical planning needs of patients and when appropriate, their support systems, and develops appropriate interventions to address such needs * Provide patient-centered care by treating patients and their support systems with respect and dignity, involving them in their care whenever possible * Provide education to other health care providers and staff, patients and their support systems on psychosocial issues, community resources, and the role of social work

    REQUIREMENTS: A Master of Social Work Degree from a program accredited by the Council on Social Work Education AND one year of full-time social work experience in a health care setting or equivalent. Experience can include a practicum placement as part of a social work training program and/or employment experience.

    LEGAL REQUIREMENT

    Within 90 days of hire must hold one of the following credentials issued by the state of Washington: * Agency Affiliated Counselor * Licensed Social Work Associate -Advanced * Licensed Social Work Associate - Independent Clinical * Licensed Advance Social Worker * Licensed Independent Clinical Social Worker
