  • Amergis Healthcare Staffing is partnered with a much respected facility in Amherst MA who is currently seeking Peer Counselors to assist them at their Independent Living Facility.

    Details of the position include:

    Location: Amherst, MA

    Duration: 13 weeks to start, but strong chance of extension

    Facility Type: Independent Living Facility

    Openings

    • 8a-6p Mon-Fri

    Start Date: Two weeks from offer

    POSITION DESCRIPTION:

    • Conduct LTSS assessments; by identifying the needs and recommending services and resources

    • Connect consumer to social services and community resources

    • Participate on the consumer's care team

    • Perform outreach

    • Advocate for the member

    • Provide LTSS care coordination

    • Initial and quarterly home visits

    • Remain current with community resources

    QUALIFICATIONS:

    • Must have knowledge of community resources and services available for people with disabilities.

    • Excellent computer skills, knowledge of MS Office and database navigation

    • Excellent communication and writing skills to ensure respectful delivery of services

    • Excellent time management and organizational skills

    • Must have valid MA driver's license and reliable transportation as well as proof of insurance if applicable.
