BayCare Health Systems is currently seeking experienced Home Health Registered Nurses to join our outstanding and compassionate Seasonal Pool team.

• 13 week assignments

• Must live more than 100 Miles from BayCare facilities to qualify

• Location: Tampa, Dunedin, Lakeland FL

• Shift Hours: Monday - Friday, 8:00am - 5:00pm and every 3rd weekend

• Home visits in assigned service area: Hillsborough, Pinellas or Polk County

The Home Health RN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include:

• Oversee the activities of the multi-disciplinary team that provides care to the patient

• Monitor the provision of therapeutic services through written and verbal communication with all disciplines

• Ensure that all visits scheduled and provided are authorized and covered by physician orders

• Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits

• Inform the scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor

Required Certifications and Licensures:

• BLS (Basic Life Support)

• RN (Registered Nurse) License

• Valid Driver's License

Education:

• Required Associate's or Diploma Nursing

• Preferred Bachelor's Nursing

Experience:

• Required 2 years RN Home Care experience
