    Description

    Performs patient care as a nurse practitioner with limited or no on-site access to a physician for guidance and oversight at the Petersburg Health Department Clinics. Clinics include STI, OB/Family Planning, Teen and Men's Sexual Health. STI and Men's Sexual Health clinics are designed to diagnose and treat sexual transmitted infections. Performs physical examinations, orders diagnostic tests, prescribes controlled substances, may directly perform or direct others to perform nursing assessments, may perform lab tests, take measurements, and develop and implement care plans including the administration of medication andcase management.

    Integrates medical knowledge, experienced-based knowledge, interview information, physical

    examination and laboratory data into comprehensive assessment of patients' health status.

    Secures and documents or reviews health and social history.

    Recognizes the diversity of individuals/populations and applies the appropriate language and communication tools; this may include but not limited to individuals from different cultural, socio-economic, sexual orientation and gender identity and expression.

    Assesses the health status and health literacy of client, including determinants of health, using multiple sources of data.

    Performs individualized assessment and physical examination based on program and public health guidelines, client needs, and clinical judgment.

    Diagnoses, treats and prescribes as indicated; Provides counseling as appropriate. Orders and interprets screening, diagnostic, and laboratory procedures. Conducts or directs continuing care activities; arranges referrals; ensures continuity of care by acting as liaison among health departments, private physicians and hospitals.

    Directs appropriate referral for patients whose needs require change in level of care. Uses data, evidence and information technology to understand the impact of determinants of health on clients.

    Maintains Board of Pharmacy Controlled Substances Registration (CSR) license in order to provide pharmacy services throughout the health district.

    Position Requirements

    Must have completed an accredited nursing/nurse practitioner education program, and possess valid Virginia license as a registered nurse, and licensed Family Nurse Practitioner with authorization to prescribe.

    Knowledge of nursing, medicine, and pharmacology. Ability to work with minimum supervision.

    Ability to obtain a comprehensive health history, perform, and/or interpret results of diagnostic tests,

    and utilize physical examination techniques.

    Possess strong verbal and written communicating skills.

    Ability to work with diverse cultural

    backgrounds needed.

    Professional CPR/AED certification.
