  • Providence Connections Palliative Care seeks a board-certified Nurse Practitioner or Physician Assistant to join their inpatient team. This well-established, trans-disciplinary team provides a coordinated program to patients and their families for the management of chronic conditions and serious, life-limiting illness. The ideal candidate will have previous palliative care experience. Primary location is Providence Portland Medical Center. Connections Palliative Care is part of a growing integrated Home and Community Services program which collaborates with Hospice, Home Health, Geriatrics as well as hospital-based practitioners to care for patients and their families.

    • Full-time, 1.0 FTE

    • Monday through Friday, daytime hours

    • Collaborate with physicians, nurses, social workers, pharmacists, chaplains

    • Minimum of one year of post-graduate clinical experience in palliative care, hospice, Geriatrics, and/or oncology care is strongly preferred
