    Clinical Coordinator -Social Worker for Medically Fragile Children in Treatment Foster Care

    Are you a social worker with a special interest in helping children with medical, physical, and developmental challenges? We're seeking a full-time Clinical Coordinator to join us in serving youth who are medically fragile and are placed in highly specialized foster homes.

    • Provide case management to a caseload of 8-10 children placed throughout central Maryland.

    Perfect candidates love driving as part of their work day!

    • Develop and oversee implementation of child's treatment plan.

    • Visit regularly with the child and foster parent both virtually and in person.

    • Provide monitoring and support to foster parents.

    • Collaborate with health care providers, courts, and school systems.

    • Forge professional relationship with Custodial Agency worker (DSS) and child's attorney.

    • Attend weekly Team Meetings and Clinical Group meetings.

    • Work in partnership with our program's nurse specialists.

    • Participate in weekly on-site supervision by LCSW-C approved supervisor.

    Qualifications:

    • Bachelor or Master in Social Work required

    • Maryland Issued Social Work License (LBSW or LMSW) is required.

    • Self-motivated and detail-oriented with ability to multi-task.

    • Valid driver's license with access to a registered and insured vehicle.
