    Job Responsibility

    • Interviews new patients and provides program orientation.

    • Conducts psychosocial assessment related to chemical/substance abuse and dependence.

    • Collaborates with healthcare team members to develop and implement patient treatment plan.

    • Conducts individual, group and family counseling sessions.

    • Provides transitional care services to facilitate patient discharge.

    • Completes documentation in accordance with policies and procedures of the organization, State and Federal regulations.

    • Serves as a liaison to referral sources to facilitate continuity of care.

    • Participates in related utilization review and performance improvement activities.

    • Performs related duties as required.

    Job Qualification

    • *High School Diploma or equivalent, required. Bachelor's Degree in human service field, preferred.

    • *Credentialed Alcohol and Substance Abuse Counselor (CASAC), required.

    • *Minimum of three (3) years substance abuse counseling experience, required.

    The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.When determining a team member's base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).

    The salary range for this position is $58,500-$79,300/year
