    At Sunrise, our Case Manager is responsible for managing the client's health and wellness as indicated by the Service Care Plan.

    RESPONSIBILITIES & QUALIFICATIONS

    Responsibilities:

    • Supervising of LPNs, HHAs and other health professionals

    • Communicating with the client's family and physicians

    • Ensuring regulatory compliance and quality improvement

    Qualifications:

    • A passion for working with seniors

    • Possess and maintain status as a Registered Nurse (New York), BSN preferred

    • 3-5 years of acute care, assisted living or geriatric care experience. Home health Case Management experience strongly preferred

    • In depth knowledge of state regulations, policies and procedures

    • Ability to work a flexible schedule and on-call as needed

    • Reliable transportation - coverage areas will be Long Island, Queens, Brooklyn, and Westchester

    • Excellent written and verbal communication skills
